How
to make a claim
If you are insured under ElderShield,
and are currently unable to perform 3 or more Activities of Daily
Living (Washing, Dressing, Feeding, Toileting, Transferring and
Mobility), please follow these 3 simple steps to make a claim:

Step 1 - Print a copy of the Claim
Form* and list of Panel
Assessors*.
Alternatively, you may obtain a copy of the Claim Form from
our Customer Service Centre at:
1 Pickering Street #01-01 Great Eastern Centre; or Contact our
Customer Service Hotline at 1800-2482888 for a copy to be sent
to you. |

Step 2 - Complete the Claim Form as best as you can.
If you are unable to do so, please have it completed
by your immediate family member or caregiver. If you still need
help, you can contact our Customer Service Hotline. |

Step 3 - Make
an appointment with an appointed assessor for the disability
assessment.
You have to: |

Pay $25 if the assessment is done at the assessor’s
clinic. |
| • |
Pay $100 if a house
call is required |
| • |
Bring the completed Claim Form,
and any available medical records or discharge summary
that you have, during the appointment |

Bring your medicine (if possible).

assessor will send the completed Claim Form together with
the results of the disability assessment to your insurer for
processing) |
